The Online Consultation Form is for people who want to embark on personal training
sessions with me or if you wish, your consultation can be done for free at DW
Fitness, prior to your first session.
This form takes less than 5 minutes to complete and provides me with all the
information required to create you the most effective exercise plan for your
goals. It also gives me the relevant information prior to your first training
session.
Please try to fill in all the questions. If you leave any questions blank, I will
assume that the question don't apply to you or that you do not know the answer to
that question at this moment in time. Always consult your GP before starting any
personal training, to ensure that you are OK to start.
